| Services Tailored to the Individual At People’s Care, we start with a comprehensive in-home Care Assessment to fully understand the person’s physical, social and cognitive needs. Based on the Care Assessment, we develop a written Individual Service Plan (ISP) that can be customized to include homemaking and personal care services. Homemaking includes: cooking, light housekeeping, medication reminders, transportation, shopping, errands and pet care. Personal Care includes: bathing, dressing, grooming, feeding, toileting, walking, transfer and exercises. Companionship includes: personalized activities that give meaning to your loved one. All services provided by People’s Care are diligently supervised by a People’s Care Professional Care Manager — a social worker or nurse and a full employee of People’s Care. When care services begin, the Professional Care Manager matches one or more of our expert Care Companions to the person’s specific health, cognitive and social needs. While a primary caregiver is assigned to ensure consistency of care, we also assign back-up Care Companions as needed to guarantee that there are no gaps in the care provided. This team approach to staffing ensures a smooth transition and seamless care. |
