Whether you need In-Home Senior Care around the clock, morning until evening, or merely a few hours several times a week, you can put your trust in People’s Care. Comprehensive Care Assessment
Our Care Process begins with a complimentary, comprehensive in-home Care Assessment (a $500 value!) conducted by a Professional Geriatric Care Manager. The purpose of the Care Assessment is to fully understand your loved one’s physical, social and cognitive needs — as well as your own concerns.

Individual Service Plan (ISP): A Roadmap to Care
As part of the comprehensive Care Assessment, your Professional Care Manager will provide you with an Individual Service Plan (ISP) that documents the support needed as well as measurable success metrics that we will work with you to achieve. The ISP focuses on your loved one’s health and well being, as well as his or her social and recreational needs, and can be customized to include the following:

Personal Care Services

  • Transport/accompany your loved one to doctor’s appointments
  • Discuss lab results and other medical issues with the family
  • Monitor and log health information, such as when medications are taken, vital signs, nutrition and exercise, if needed
  • Monitor behavior, watching for trends and/or changes
  • Provide resources and support to your family
  • Provide 24-hour emergency contact
  • Be the eyes and ears of you and other family members

NOTE: All Care Management activities are performed or carefully supervised by your Professional Care Manager, who is a social worker or nurse.

Continuous Monitoring by Your Professional Care Manager
When care services begin, your Professional Care Manager will match one or more of our expert Care Companions to your loved one’s specific health, cognitive and social needs. Your Professional Care Manager will also fully prepare the Care Companion with instructions on how to implement the written ISP. Care Companions are supervised and monitored by your Care Manager, including unannounced visits and ongoing client-specific training.

While a primary caregiver will be assigned to your loved one to ensure consistency of care, we also assign back-up Care Companions as needed to guarantee that there are no gaps in the care provided. This team approach to staffing ensures a smooth transition and seamless care.

About People’s Care
At People’s Care, we work in partnership with you and other family members to ensure consistency in the support services provided. Your People’s Care Professional Care Manager is available 24 hours a day, on an on-call basis, and includes rapid response to emergency situations. Not only do we provide private in-home Senior Care, we also provide Rehabilitation Care, Chronic Illness Care, and care for those with Alzheimer’s/Dementia and Challenging Behaviors. Whether you need assistance around the clock, morning until evening, or merely a few hours several times a week, you can put your trust in People’s Care.
