Our Care Process

Our Care Process begins with a complimentary, comprehensive Care Assessment (a $500 value!). The Care Assessment is conducted by a People’s Care Professional Care Manager, who is a social worker or nurse and employee of People’s Care. The purpose of the Care Assessment is to fully understand your loved one’s physical, social and cognitive needs — as well as your own concerns.

During the in-home Care Assessment, your People’s Care Professional Care Manager will gather the following information into one document:
  • Family history
  • Health considerations
  • Current medications
  • Daily care needs
  • Social and recreational needs
  • Behavioral considerations
This comprehensive Care Assessment may also include a home safety inspection and cognitive screening, as needed. The assessment process requires approximately two hours.

Individual Service Plan (ISP): A Roadmap to Care
Based on the comprehensive Care Assessment, your Professional Care Manager will create an Individual Service Plan (ISP) that documents the support needed as well as measurable success metrics that we work with you to achieve. The ISP focuses on your loved one’s health and well being as well as social and recreational activities, and will be customized to include the following services:

  • Light housekeeping
  • Laundry
  • Shopping & errands
  • Medication reminders
  • Transportation
  • Pet care

Personal Care Services

  • Bathing
  • Dressing
  • Grooming
  • Feeding
  • Toileting
  • Walking
  • Exercise
  • Transfer
Care Management
Senior Care Management    
  • Transport and accompany your loved one to doctor’s appointments
  • Communicate doctor visit results with the family
  • Ensure follow up lab tests and appointments are completed
  • Monitor and log health information, such as when medications are taken, vital signs, recuperation progress, nutrition and exercise
  • Monitor behavior, watching for trends and/or changes
  • Provide resources and support to the family
  • Provide 24-hour emergency contact
  • Act as the eyes and ears of family members
NOTE: All Care Management activities are performed or carefully supervised by your Professional Care Manager, who is a social worker or nurse.

Continuous Monitoring by Your Professional Care Manager
When care services begin, your Professional Care Manager will match one or more of our expert Care Companions to your loved one’s specific health, cognitive and social needs. Your Professional Care Manager will also fully prepare the Care Companion with instructions on how to implement the written ISP. Care Companions are supervised and monitored by your Care Manager. At People’s Care, we work in partnership with you and other family members to ensure consistency in the support services provided. Your People’s Care Professional Care Manager is available 24 hours a day, on an on-call basis, and includes rapid response to emergency situations. Whether your loved one needs assistance around the clock, morning until evening, or merely a few hours several times a week, you can put your trust in People’s Care.
