Care Management: The People’s Care Difference
All services provided by People’s Care are diligently supervised by a People’s Care Professional Care Manager. The Professional Care Manager is a social worker or nurse and an employee of People’s Care. He or she is your main point of contact throughout the entire Care Process. From conducting your free, in-home comprehensive Care Assessment, to adjusting your loved one’s Individual Service Plan (ISP) as the need for care changes, your People’s Care Professional Care Manager is with you every step of the way.
 
Comprehensive Care Assessment
During this free, in-home comprehensive Care Assessment (a $500 value!), your People’s Care Professional Care Manager’s goal is to fully understand your loved one’s physical, social and cognitive needs — as well as your own concerns. He or she will gather the following information into one document:
  • Family history
  • Health considerations
  • Current medications
  • Daily care needs
  • Social and recreational needs
  • Behavioral considerations
  CareManagement
This comprehensive Care Assessment may also include a home safety inspection and cognitive screening, as needed. The assessment process requires approximately two hours.
4 Hours of in home care-Free Individual Service Plan (ISP)
Based on the comprehensive Care Assessment, your Professional Care Manager will provide you with an Individual Service Plan (ISP) that documents the support needed as well as measurable success metrics that we will strive to achieve. The ISP focuses on your loved one’s health and well being and will include social and recreational activities. Together, you, your loved one, and your Professional Care Manager will determine the best road map for what to do now — and as the need for care changes over time.

Care Management Supervision
When care services begin, your Professional Care Manager will match one or more of our expert Care Companions to your loved one’s specific health, cognitive and social needs. Your Professional Care Manager will also fully prepare the Care Companion with instructions on how to implement the written ISP. Care Companions are supervised and monitored by your Care Manager, including unannounced visits and ongoing client-specific training.

While a primary caregiver will be assigned to your loved one to ensure consistency of care, we also assign back-up Care Companions as needed to guarantee that there are no gaps in the care your loved one receives. This team approach to staffing ensures a smooth transition and seamless care.

Attending Physician Appointments
Your Professional Care Manager, at no cost, will attend all physician appointments with your loved one and report the results back to you and other family members upon request (a $100 value!). He or she will also assist your loved one in preparing for the appointment by creating a list of questions, collecting data the physician has requested, recording the physician’s instructions and ensuring careful follow up.

Your Emergency Contact
Your Professional Care Manager is your point of contact for all emergencies, 24/7. Should your loved one require an emergency room visit, your Professional Care Manager will be there as an advocate and to keep you informed if you cannot be there yourself.

Coordinating Services
Your Professional Care Manager will also help identify and locate other resources for your family and coordinate all care and support provided. In this way, overlapping services are avoided, saving you money and improving the quality of care provided to your loved one.
